Enrollment and Forms

Parents of students who are Profoundly Mentally Handicapped and want their child to attend classes at Merrick should speak to the staffing specialist assigned to their student's current school.

Students who are served in the Alternative Telecommunication program are referred by the Miami-Dade County Public Schools Division of Alternative Education.

The Hospital/Homebound Instructional Program (HHIP) serves students who are confined to the home or hospital due to illness. Parents of students who require these services must submit the Application for Homebound / Hospitalized Instructional Program signed by the treating physician. The parent must also sign this document as well as a Consent Form for the Mutual Exchange of Information . It also is important that parents review the Summary of Procedural Safeguards for Students with Disabilities. These documents can be downloaded using the links below and are available in English, Spanish and Haitian Creole. After these forms are completed and submitted to Merrick 's Student Services office, parents are contacted and invited to attend a meeting to determine the student's eligibility for these services. If the student is eligible, an Individualized Educational Plan (IEP) will be developed.
